Donors Annual Return Form
The information sheet below contains some background about the financial disclosure obligations for donors and provides guidance for completing the forms.
The donor return forms may be downloaded in Word or PDF format. For detailed information, refer to the Guide.
The Word format may be completed 'on-screen' and the form printed or saved as appropriate. This form, with any electronic attachments, may be lodged by email to fad@aec.gov.au. If emailing the return, a signed form must also be sent by post.
The PDF version may be printed as a paper form only for lodging by post. It cannot be completed 'on-screen'.
